Reports and Complaints Form
Please use the form below to notify ZiuZ in case you (as an external stakeholder) have experienced or observed one of the following situations:
- Accidents
- Inappropriate behaviors
- (Threat of) violence
- Other violations of the Code of Conduct and Ethics.
If you’re an internal stakeholder, please report inappropriate behavior to either a supervisor, ZiuZ’s QA Manager, or ZiuZ’s external counselor.
